CMS Revises HQRP to Increase More Patient Visits

The Muse solution was created to address the reoccurring issue of the end-of-life gap in care in hospice. Muse’s machine learning technology ensures that every hospice patient transitions with exceptional dignity, comfort and attention. Muse’s predictive analytics model is designed to identify patients that are likely to transition and empower clinical staff with the necessary insights to provide regular visits and exceptional care to patients in the last days of life.

According to the Centers for Medicare & Medicaid Services (CMS), the organization plans to revise the Hospice Quality Reporting Program (HQRP) measures and the Hospice Item Set (HIS) to add the Hospice Visits in the Last Days of Life measure. This measure further reaffirms Muse’s core principal to increase care to patients in critical condition.

Analyzing the new measure

CMS now recommends visits by registered nurses or social workers should occur in at least two of the last three days of life. The reasons behind this decision is due to the patient’s needs to receive more intensive services in the last days of life. As a result, CMS expects care would increase in order to meet the increases in clinical, emotional, and spiritual needs of the patient and family.

How Muse can help

Muse uses deep learning neural network based modeling to analyze hundreds of thousands of data points from multi-disciplinary visits. This data empowers clinicians to identify at a very high level of accuracy those hospice patients that are likely to transition within 7-12 days. This in turn drives more accurate communication for patients and their families. By using Muse’s Care Review hospice utilization tool, agencies are able to define upper and lower multi-disciplinary thresholds by risk score and are alerted when patients are either below (under-utilized) or above (over-utilized) those defined limits. Ultimately, patients in the most critical stage receive the care they need by virtue of the insight gleaned from the Muse application.

These new measures will require that hospice providers serve patients at a greater rate than ever before. More hospices and clinicians realize the benefits of increased care to at risk populations. Allow Muse to drive those interactions and identify these high-risk patients for you. Interested to learn how Muse can help your agency? Click the button below to contact us today:
